Our self-assessment tool is designed to help you stay active, maintain your independence as you age, and reduce your risk of falling.

You will be asked a series of questions, which should only take a few minutes to complete. You can take the online self-assessment yourself or ask someone you know to help.

Please answer as many questions as possible to help make your personal action plan as helpful as possible.

Once completed, you will receive your personalised action plan. Your action plan can be downloaded, printed and shared with friends, family or health professionals.
