The Community Gateway has been launched to provide access to a range of voluntary sector support. Cornwall's charities and community groups have combined forces to support people. It provides a ‘gateway to independence’, working with people to identify their needs and put appropriate packages of support in place.
The Gateway aims to create personalised plans that prevent hospital admissions, reduce social isolation, and improve wellbeing. It will also connect people to mental health support, community activities and wider winter support, including hot food and warm spaces.
The Community Gateway is a dedicated telephone line available for everyone and provides access to a wide range of professional voluntary sector support. Open 0800 - 2000, seven days a week, 365 days a year.

Ageing Well is a priority for the NHS, and the national NHS Ageing Well programme focuses on how we care for our population in the community. Preventing avoidable hospital admissions is not just about bed management and managing increasing demand, as important as these are.
But research is clear that even a short stay in hospital leads to physical and mental deconditioning, particularly for older people. Deconditioning refers to changes in the body that occur after a period of inactivity, such as bed rest or a sedentary lifestyle. It can lead to people finding it difficult to accomplish normal daily activities and remain independent

Cornwall Falls Service are a small team of highly qualified, experienced falls practitioners and falls technicians who: provide individual assessment and treatment for clinically complex people who have recurrent falls.
Our aim is to work collaboratively with people who fall to reduce the risk of injury and maximise their ability to stay steady by ageing well. Our criteria are a person over the age of 50 with 1 of the following: a clinically complex person who has recurrent falls, who has had a multi-factorial falls risk assessment completed, interventions actioned and remains at risk OR an upper limb fragility fracture following a fall and a history of 2 or more falls in the last 12 months.
